How do I refer a patient?
You can refer patients through our HIPAA-compliant referral form or by contacting our office directly. We also offer a secure online portal for DICOM upload, case sharing, and report access.

Why was I referred to Revive Dental Support Studio?
Your dentist or specialist referred you to us for imaging or digital records that are needed to plan your care. We provide advanced diagnostic services like 3D scans and digital impressions to help your provider offer the best treatment possible.
Will I receive treatment at your studio?
No. We do not perform dental treatment. Our job is to support your dentist with imaging, scanning, or digital planning. Once your visit is complete, we send the information back to your provider.
What should I bring to my appointment?
Please bring your photo ID, insurance information (if applicable), and any referral forms or instructions from your dentist. If you have previous scans or dental records, bring those as well.
Is the imaging safe?
Yes. We use low-dose, high-resolution CBCT technology that meets all safety standards. Our team follows strict protocols to ensure minimal radiation exposure.
How long does the appointment take?
Most appointments last 15–30 minutes, depending on the service provided. You’ll receive instructions during scheduling so you know exactly what to expect.
Will my dentist get the results?
Yes. All imaging, scans, and reports are securely sent to your referring dentist within 1–2 business days.
Can I get a copy of my scan or images?
Absolutely. We can provide you with a digital copy upon request, or send it directly to your provider or dental lab.
