Appointments
All visits to Revive Dental Support Studio are by referral only. Your dentist or specialist will coordinate your visit with our office, or you may be contacted directly to schedule a time. If you need to reschedule or have questions before your appointment, feel free to call us at [Your Phone Number].
Patient Forms
To save time at your appointment, please complete your intake forms online prior to arrival. Your dentist may send your forms directly through our secure portal, or you may receive a link via email or text from our office.

Payment Options
We accept payment in two convenient ways:
• At our studio at the time of your appointment (credit/debit card, HSA/FSA accepted)
• Through your referring dentist’s office, if they have made prior billing arrangements with us
If you’re unsure which method applies to you, just give us a call—we’re happy to clarify.
Dental Insurance
Please note: We do not bill dental insurance directly.
Some of our services may be eligible for reimbursement depending on your insurance provider. Upon request, we can provide you with a detailed receipt and procedure codes so you can submit a claim on your own.
